Physical Therapist (PT) at Interim HealthCare summary:

The Home Health Physical Therapist (PT) provides in-home therapy services to patients in need of rehabilitation. This part-time position requires a licensed Physical Therapist in Colorado with at least one year of experience, preferably in home health care. Responsibilities include conducting assessments, delivering therapeutic treatments, and collaborating with healthcare team members for optimal patient care.

Home Health Physical Therapist

  • Location: Brighton, Thornton and Northglenn area
  • Pay Rates: $75-$150 per visit, (depending on visit type), PLUS mileage and travel time compensation
  • Status: Part-Time

     

We are looking for someone energetic, self-sufficient, hardworking, and reliable to provide care for clients in their home. Someone who is has the ability to multi-task, work under pressure with changing priorities, and effectively handle a heavy workload. Must be familiar or willing to learn Oasis documentation.

Responsibilities

  • Complete initial and ongoing comprehensive assessments as assigned.
  • Provide interventions and therapeutic treatment
  • Effectively communicate with other members of the interdisciplinary healthcare team to promote the coordination of patient care and planning for discharge.
  • Participate in Quality Assessment and Performance Improvement (QAPI) program.
  • Responsible for reporting on the condition of patient to the Rehab Director, and other appropriate staff members.

Required Qualifications

  • Current Physical Therapist (PT) licensure in the state of Colorado
  • Current Basic Life Support (BLS) CPR or willing to obtain upon hire (demonstration course required)
  • One year of PT experience
  • Home health care experience preferred, but not required
  • Pass a background check and drug test (we do to test for THC)
  • No Covid Vaccination Required
