We provide a team-based model of care including physicians, RNs, LPNs, Home Health Aides, physical/occupational therapists, social workers, transportation, a day center team, and more We are privileged to serve patients who are 55 years of age and older, helping them delay/avoid nursing home placement and remain living in the community as long as possible. PHYSICAL THERAPIST - FULL-TIME EDDY SENIORCARE - Latham, NY. This program is located on the beautiful 83 acre, hillside campus of the St. Joseph Provincial House. Eddy SeniorCare has a rewarding opportunity for full-time Physical Therapist responsible to manage a caseload of patients requiring physical therapy, both restorative and maintenance. The focus is on outcomes, not on productivity What you will do: Work as a part of a team to manage a caseload of patients including a mix of those who require post-acute or subacute rehab as well as those benefitting from maintenance therapy. Care will be provided both in our rehab gym as well as in the home. Evaluates and treats individuals with functional deficits secondary to neurologic, orthopedic and medical conditions.
