Job specification

  • Coordinate and manage care for patients, ensuring that appropriate services and resources are provided.
  • Collaborate with physicians, social workers, and other healthcare professionals to create individualized care plans.
  • Assess patients' needs and identify available resources, ensuring the effective delivery of services.
  • Monitor patient progress and adjust care plans as necessary to improve outcomes.
  • Educate patients and families on care plans, treatment options, and healthcare resources.
Candidate requirements A Bachelor of Science in Nursing (BSN) is required; a Master's degree in Nursing (MSN) or related field is preferred. Current Registered Nurse (RN) license in the state of Texas. Minimum of 3 years of clinical nursing experience, with at least 2 years in a case management or care coordination role. Case Management Certification (CCM) preferred. Basic Life Support (BLS) certification is required.
