We have per diem shifts available now in our Long-Term Care and Rehab Facilities - What You'll Do: As an LPN Admissions Nurse, you will play a pivotal role in the admission process, utilizing your nursing expertise to assess, plan, and coordinate the transition of residents into our facility. Your compassionate care will ensure a smooth and comfortable entry for those in need of our services. Key Responsibilities: Conduct comprehensive nursing assessments for potential residents Collaborate with healthcare teams to review medical histories and care needs Assist in developing individualized care plans for new admissions Coordinate and communicate care plans with residents, families, and healthcare professionals Educate residents and families about facility services, procedures, and care expectations Ensure compliance with regulatory requirements during the admission process If you are a compassionate and experienced nurse with a talent for guiding individuals through healthcare transitions, join our team as an Admissions Nurse.
