The Community Health Worker builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support, and advocacy. Our regular business hours are Monday-Friday 8:00am-4:30pm, though the department provides staff with flexibility in effort to strive for a desired work/life balance. Essential Duties and Responsibilities Qualification Requirements To perform this job successfully, an individual must be able to perform each essential duty and responsibility satisfactorily. Regular attendance according to the position s management approved work schedule is required for all positions. Build trusting relationships with families. Serve as liaison between health/social services and the community. Remove barriers that prevent community members from accessing certain health and social services. Provide culturally and linguistically appropriate services to assigned clients. Conduct intake interviews with clients, including enrolling and/or referring clients into additional county programs. Make regular home visits to clients to support their health and well-being. Work together with clients to develop a wellness plan. Coach and support clients to set and accomplish wellness goals. Facilitate appointments with health and social service organizations. Follow up on appointments or other instructions from staff and providers by call, text, or home visit. Work to ensure the client attends and has access to scheduled appointments. Participate in client case conferences and work with Public Health Nurses regarding caseload. Identify and reinforce risk reduction behaviors. Provide appropriate education utilizing a state defined curriculum. Perform risk assessment, informal counseling, social support, and advocacy to clients. Collaborate with other agencies in making appropriate resource referrals. Help coordinate client transportation and accompaniment as needed, to scheduled appointments. Advocate for the needs of clients by assisting them in effectively communicating with healthcare or social service providers. Interpret, translate, or provide cultural mediation related to health services or information for clients. Act as a patient advocate and liaison between client/family and community service agencies. Participate in community public education activities and outreach to raise awareness. Deliver health-related pr ventive services such as blood pressure, glaucoma, and hearing screenings. Distribute flyers, brochures, or other informational or educational documents to intended communities. Teach classes or otherwise disseminate medical or dental health information to school groups, community groups, or families/individuals, in a manner that is culturally appropriate. Provide ongoing follow up and outreach. Complete regular data entry per department or program guidelines. Assist clients in completing all required forms, agreements, case notes, etc., and supply copies of the information to clients when appropriate. Keep proper and accurate documentation organized and entered in appropriate system. Serve as an agency representative on community committees/task forces as identified by the designated lead or supervisor. Follow all Agency Policy, Rules, and Statutes. Implement and carry out assignments from the designated lead or supervisor. Attend training courses as they relate to position, populations served, or other identified needs. Participate in positive community relations, training, and planning. Assist community organizations in program development related to Health and Human Services. Maintain working knowledge of Agency programs specific to assigned program areas which may include: HuBERT, PH Doc, REDCap, MIIC, FAP, MDH Partnerlink, MNTrac, CATCH, and MEDSS, and other state/federal computer information systems to ensure appropriate support for all functions within the Unit. Maintain working knowledge of other software programs such as: Word, Excel, PowerPoint, SharePoint, Teams, Outlook, Alertcast, OnBase and other specialized software packages. Respond 24/7 as needed during a Public Health Emergency Response. Other duties as assigned per the lead or supervisor.
