CHI Health would love to add a Clinic LPN to our Valley View Priority Care (Urgent Care) / Family Medicine clinic located in Council Bluffs, IA 

**IF you have completed your Medical Assistant Degree, are an RN (Nurse) or have your EMT / Paramedic and are interested Please APPLY We are open to hiring all titles Sign on bonus will apply to these titles as well 

Hours: Monday-Friday 8a-8p (3-12s) rotating every 3rd weekend & holidays

Our full-service network provides a variety of Primary Care & Priority Care Services, including Family, Internal, Geriatric, and Pediatric Medicine, in addition to several specialties to deliver custom care based on the unique needs of our patients.

Responsibilities

**$5000 Sign On Bonus Offered**

Paid out in FULL after 30 days of employment

This position will support our healthcare teams by ensuring they have everything they need in order to care for our patients. They also play a huge role in a patient’s experience and prep when they visit any of our clinics. Expectations:

  • Performs general nursing care to patients by following established clinic workflows.
  • May perform specific nursing care as it relates to specialty of the practice.
  • Obtain patient consent for care and ensure patient confidentiality.
  • Obtain and document health history, information on chief complaint, vital signs, and health risk factors and assesses disease condition.
  • Provide patient care based on practice guidelines, standards of care, and federal/state laws and regulations.
  • Execute prescribed treatments and medical interventions; administer prescribed medications, and monitors and documents treatment progress and patient response.  This includes but is not limited to preparing and administering injections, performing routine tests, treating wounds, and changing bandages.  Refer to/consult with physicians, other health providers, and community resources to prevent/resolve problems or concerns.  
  • Educate patients and families about health status, health maintenance, and management of acute and chronic conditions.
  • Document patient assessment and intervention data using established medical record forms/automated systems and documentation practices.  
  • Participate in multidisciplinary teams to improve patient care processes and outcomes.  
  • Track quality assurance data and monitor for acute and chronic care management.
  • Performs other duties within scope of role as assigned.
Qualifications

Minimum Qualifications:

  • Graduate of an accredited Licensed Practical Nursing program
  • Current LPN license in good standing with the state licensing board 
  • Basic Life Support (BLS) – American Heart Association 
  • Completion of IV Therapy Course
