Texoma Community Center (TCC) is one of 39 Texas Community Centers, which are governmental entities as defined by Title 7 of the Texas Health and Safety Code.

Position Summary: This is a QMHP level position for individuals working towards clinical licensure in either Social Work or Counseling. This position provides intake, screening, and assessment for individuals recently discharged from our local behavioral health hospitals or who were referred for LOC-5 services in compliance with clinical guidelines. To provide continuity of care patient, impact monitoring and service assessment provision and monitoring. Provides service coordination and brief interventive treatment to clients of all ages with behavioral health issues who have been referred to TCC, received any TCC services, recently experienced a crisis episode, and/or discharged from psychiatric hospitals to TCC's catchment area. To provide brief crisis intervention, psychosocial rehabilitation, and skills training in the form of individual or group sessions and follows the U/M guidelines and TAC. To make appropriate referrals to other programs or services based on individual needs from assessments in accordance with the philosophy, goals and objectives of Texoma Community Center. This position is the primary overseer of the CARE and CMBHS data system and data entry for error correction, COR, PPB, SFAF, and Agency TRRs. This position also serves as the primary back up for the care coordinator. Operates under trauma-informed and recovery-oriented models of care which align with CCBHC standards.

Qualifications:

Education and/or Experience:

  • Master's Degree in Social Work, Sociology, Psychology or Counseling required
  • Two (2) years Human Services Experience required
  • Experience working with individuals with mental illness and/ or addiction issues desirable
Certificates, Licensure, Registrations:
  • Current driver's license and vehicle for use at work.
  • Actively seeking licensure as either a Licensed Professional Counselor, Licensed Clinical Social Worker, or Licensed Marriage and Family Therapist.
Knowledge, Skills and Abilities:
  • Ability to speak a second language relevant to population served preferred
  • Must be self-motivated, organized and detail oriented
  • Knowledge of Microsoft Office Suite
  • Consistently practices Trauma-informed Model of Care, understands and considers the pervasive nature of trauma.
  • The knowledge of cultural differences and ability to work with individuals from different cultural backgrounds.
