Rehabilitation Counselor

Global Healthcare Services (GHS) is currently looking for hardworking, reliable, energic and dynamic individuals to become members of our agency team. Becoming a Rehabilitation Counselor, you will be responsible for providing rehabilitation services to adults and children to ensure their self-sufficiency and restoration as outlined on their Individualized Treatment Plans. Hours will vary based on the employee’s caseload.

In this position employees will be expected to provide supportive living services, supportive counseling, facilitate progress on all client’s goals as listed on each client’s treatment plan and work with clients to reintegrate them into the community.

Duties

  1. Face to face contact with clients providing 4 monthly visits for children and 6 monthly visits for adults.

  2. Outreach

  3. Communicate and coordinate with the Program Director on the client’s Progress.

  4. Complete in SharedNotes daily contact notes/sheets, monthly progress notes, and all other notes for each client.

  5. Provide all necessary supportive services that ensure client’s well-being, safety, and meaningful community integration.

  6. Provide transportation to clients for all appointments (medical, therapeutic, etc.)

  7. Communicate with Therapist if client is unreachable after 24hrs of an attempted visit or phone call.

  8. Other duties as assigned.

Pay: Contractual
