Social Worker 2

- Assists in monitoring status of individuals presently waiting of a regional psychiatric hospital (RPH) bed after an order from the court.
- Collaboration with jail mental health staff, assess for changes in acuity that could move client higher on priority waitlist.
- Travel to jails as needed to meet with those awaiting RPH beds, collect information from jail mental health staff and liaison between jail staff and RPH staff.
- Performs as lead worker in collaboration with social worker 1 in the review and development of client care.
- Coordinates assessment, evaluation & social service needs as identified by clients(s) on the individualized service plan (ISP) or treatment plan of clients throughout the continuum of care cycle.
- Assess client's progress towards competency restoration and competency to stand trial.
- Provides intervention with acute, complex and varied-discharge needs of clients.
- Manages cases containing numerous problems and interventions & provides crisis situational counseling and interventions with various modalities including CBT.
- Participates in process of establishing, implementing & directing social services programs & treatment system objectives.
- Develops & designs treatment intervention techniques & procedures.
- Evaluates & modifies treatment systems to be responsive to client's progress & ability to change behavior.
- Complete and maintain necessary documentation/records to satisfy federal, state, and local agencies and accrediting bodies. Document all findings and actions according to RPH, OhioMHAS, and DAS policies.
- Plans & implements continuity of care from hospital to community by functioning as linkage for treatment team with case management system through ongoing regular collaboration.
- Establish and maintain working relationships with other facilities, agencies, and support services in order to coordinate referrals and follow up.
Unless required by any applicable union contract and/or requirements of the Ohio Revised Code, the selected candidate will begin at Step 1 of the pay range schedule listed above, with an opportunity for pay increase after six months of satisfactory performance and then a yearly raise thereafter.

Qualifications
Requires current license as independent social worker (i.e., LISW) as issued by State of Ohio Counselor, Social Worker & Marriage & Family Therapist Board in accordance with ORC 4757.27.
