Job Title: Case Manager (Exempt) Reports To: Director of Therapeutic Services Function: Under the supervision of the Director of Therapeutic Services, the Case Manager integrates all services provided by Hope Village for Children for each child and family in their caseload. With a focus on trauma-informed care and crisis management, Case Managers ensure that each child receives comprehensive support tailored to their individual needs. They advocate for the children and monitor the effectiveness of their care and treatment plans, collaborating closely with various stakeholders to promote positive outcomes. Major Duties: Orient and integrate new children into the Hope Village for Children program. Initiate services, identify gaps, and make referrals to minimize service deficiencies. Collaborate with the Director of Education to develop appropriate educational plans. Coordinate services and make referrals for medical, dental, psychological, educational, vocational, independent living, and recreational resources. Share information and collaborate with cottage and supervisory staff to ensure the well-being of each child. Promote positive relationships between staff and children. Monitor and document independent living skills development within the cottages. Facilitate children's participation in goal-setting and transition to permanency. Provide individual attention and instruction based on each child's needs and treatment objectives. Ensure timely completion of all paperwork in compliance with regulatory agencies. Consult with childcare staff and participate in case conferences and team meetings. Facilitate Family Team Meetings and communicate progress or regression with regard to goals and permanency. Maintain relationships with residents' teachers and other school personnel. Attend staff meetings, case conferences, and in-service training sessions. Act as a liaison with relevant government departments and prepare necessary reports. Assist with recreational activities on and off-campus as needed. Perform Developmental Coach duties when assigned. Participate in on-call rotation. Establish and maintain effective working relationships within the organization. Maintain confidentiality regarding agency matters and resident information. Perform other duties as assigned by the supervisor. Qualifications: Education: Bachelor's degree in Social Work or related field required. Master's Degree in Social Work (MSW) and licensure preferred. Experience, Skills, and Abilities: Minimum of two years' work experience with children and/or youth and their families. Ability to handle conflict and difficult situations. Proficient documentation skills. Ability to facilitate meetings with residents, families, and licensing agency representatives. Completion of mandated training, including CPR, First Aid, and MANDT certifications. Ability to learn in various settings and work independently or as part of a team. Strong written and verbal communication skills. Valid Driver's License and insurability through the company's automobile insurance carrier. Ability to pass background checks, drug screenings, physical exams, and other screenings as required by licensing agencies.
