Home Care Social Worker

Qualifications

  • 2+ years of experience in Family Therapy/Counseling; 2+ years of Supervised experience in the Health field
  • Master's in Social Work from a graduate School of Social Work
  • Indiana Licensed Clinical Social Worker (LCSW)
  • Microsoft Office/Suite proficient
  • PC / IPAD proficient
  • Solid assessment, clinical, and documentation skills
  • Patient oriented
  • Great interpersonal skills
  • Excellent communication skills (written and verbal)
  • Strong attention to detail
  • Highly organized

Responsibilities

  • Contribute to patient and family counseling and education
  • Participate in ongoing interdisciplinary comprehensive assessment
  • Assess patient/family psychosocial response to the illness and evaluate coping skills
  • Assist patient, family and agency staff in utilizing community resources
  • Assist team members in understanding the significant social and emotional factors related to the care of the patient and family
  • Coordinate with team members to plan and implement the home care plan of care
  • Participate in interdisciplinary team meetings
  • Maintain accurate, thorough and professional written records, and charts relevant information in a timely fashion
  • Develop and implement the discharge plan

Compensation

  • Reimbursement per visit and mileage
  • PRN Position with great flexibility
