Responsibilities

  • Provide personal care and assistance with activities of daily living (ADL's) for patients in their homes
  • Assist with medication intake and prescribed exercises
  • Perform incidental household services essential to patient health care
  • Follow the Plan of Care and make necessary updates during scheduled visits
  • Ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care

Qualifications

  • Must be a certified nurse's aide (CNA)
  • Maintain a valid driver's license, good driving record and reliable automobile
  • High school graduates or GED preferred
  • Ability to work within a multi-disciplinary team setting
  • Demonstrate the ability to work in field situations
