Primary Function/General Purpose of Position The Licensed Practical Nurse is a multi-skilled clinical professional who works within the scope of practice to assist in patient care under the direction of the provider. The LPN helps patients by providing information, services, and other assistance to support physicians and other providers in rendering patient care. Essential Job Functions Contributes to the completion of the patient’s medical documentation including but not limited to chief complaint, history of present illness (HPI), vitals, allergy and medication list, history, review of systems (ROS), etc. and documentation of patient needs Collects and processes patient specimens for various laboratory and point-of-care test (POCT) Administers ordered medications and/or vaccines via oral, injection, topical routes of administration, within the role, scope and state guidelines Manages patient requests including My Chart requests incoming patient calls and HER messages (i.e. EPIC in-basket) Assists in procedures and tests under the guidance of the Provider Participates in planning, educating, and implementing care for patients within the scope Participates in promotion of positive culture and teamwork Demonstrates excellence in care in all interactions, for both patients and internal teams Identifies opportunities for improvement, troubleshoots issues and/or communicate/escalate timely to Provider, direct supervisor, or other appropriate internal resources Maintains a safe and supportive environment by keeping the work area, exam rooms, and equipment in clean, orderly, and safe manner Maintains clinical and administrative skills to meet patient care and daily operational needs Understands and adheres to the legal responsibilities and requirements within the role Adjusts work hours and is open to travel to other urgent care facilities if necessary to meet patient care needs as directed by the manager Performs cross-functional duties as needed, including front desk duties such as registration, scheduling, answering phones, cash handling and other duties such as scribing, ordering of supplies, ect. Licensing/Certification Practical Nursing Certificate/Licensed Practical Nurse (LPN) (required) BLS Basic Life Support (required) Work Experience 1 year of experience in acute care setting (required) Training EPIC Electronic Health Record (preferred) Patient Population Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served on his or her assigned unit. Demonstrates knowledge of the principles of growth and development of the life span and possesses the ability to assess data reflective of the patient's status and interprets the appropriate information needed to identify each patient's requirements relative to his or her age, specific needs and to provide the care needed as described in departmental policies and procedures.
