Our client in Atwater, CA seeks a dedicated, passionate Physical Therapist to provide care to patients in the Atwater/Merced, CA area. Experience as a Home Health PT is preferred but if you are in the beginning stages of your career with less than a year experience, you are still welcome to apply. POSITION DETAILS: Pay: $3,714 per week. (Includes $1,162 in stipends) Location: Atwater/Merced, CA Assignment Length: 26 weeks Schedule: 4x10s. Flexible schedule (Mon-Thurs, Tue-Fri, Wed-Sat) Transportation: Reliable transportation required. Mileage reimbursed at $0.67 per mile. Travel between homes is approximately 75-100 miles per week. JOB DUTIES: Obtains all pertinent medical history from patient, family, or significant others Performs PT assessment visit and documents accordingly Performs home safety check and environmental assessment of the patient’s home environment Helps develop an appropriate and effective PT Plan of Care (POC) to be submitted to the physician for approval and implementation Functionally reassess the patient for on-going therapy Determines medical necessity for other services Evaluates the patient’s ADL and iADL abilities Effectively communicates ongoing with patient and family regarding the POC and progression of such Effectively instructs patients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc) Effectively communicates with other disciplines in the case Communication with the patient’s physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests Submits ALL OASIS assessments/documentation within 48 hours of the OASIS visit Participates in staff meetings and in-services Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care physical therapy and the benefit of the patients serviced Other duties as assigned REQUIREMENTS: Current CA Physical Therapist License Current BLS 1 year Home Health experience.
