Description of Responsibilities

The Infusion Nurse Practitioner is responsible for providing nursing services to assigned patients in accordance with the patient’s plan of care.

Responsibilities:

· Responsible for the clinical record of each patient receiving nursing care.

· Assures that progress reports are made to the physician for patients receiving nursing services when the patient’s condition changes or deviations from the plan of care.

· Adheres to the facility’s policies and procedures, including patient care policies.

· Performs initial patient evaluations/assessments and develops a written plan of care.

· Regularly re-assess the patient’s condition and nursing needs and update the plan of care.

· Furnishes those services requiring substantial and specialized nursing skills.

· Initiates appropriate preventive nursing procedures.

· Prepares clinical and progress notes, coordinates services, and informs the physician and other personnel of changes in the patient's condition and needs.

· Counsels the patient and family in meeting nursing and related needs.

· Demonstrates competency in providing patient care and provides care in accordance with professional standards and the state’s nurse practice act.

· Initiates case conferences and communicates the patient plan of care with team members.

· Evaluates and re-evaluate the need for services.

· Provides appropriate patient education and teaching materials.

· Participate in the Performance Improvement program as requested by the Performance Improvement Coordinator.

· Participate in committees when requested.

· Attends continuing education programs and staff meetings as appropriate to job responsibilities.

· Represents the facility positively and professionally.

· Perform other duties as assigned by supervisor.

Minimum Qualifications

· Graduate of an accredited school of nursing and current NP state license.

· At least one year of nursing experience, infusion nursing experience is preferred.

· Effective interpersonal, time management, and organizational skills.

· Strong communication skills in speaking and writing.

· Organizational skills sufficient to maintain consistently accurate records.

· Ability to recognize and direct information to the appropriate health care provider.

· Computer skills that include word processing and efficient use of the internet and e-mail.

· Current CPR card, health certificate, valid driver’s license, reliable transportation, automobile insurance.
