Rehab Case Manager Responsibilities include:

  • The Nurse Case Manager is responsible for coordinating patient care and ensuring that healthcare services are delivered efficiently and effectively
  • This role involves working with patients, families, healthcare providers, and insurance companies to develop and implement comprehensive care plans
  • The Nurse Case Manager will assess patients' needs, monitor progress, and advocate for patients' well-being to achieve optimal health outcomes
Requirements of Rehab Case Manager:
  • RN-2 Years Experience
