Client location and schedule hours:
Client locatd in Spring Hill . Schedule is M-W-F 9 am to 2 pm

Job Responsibilities:

Diagnoses: Diabetes, Congestive Heart Failure, Incontinence issues (laundry every day), Neuropathy in legs, Stage 4 Renal Failure, and Gout, Depression. Allergies: None know for food or medications

Goals & Objectives: Ask if meds were taken, (CG will not be there at med times) Healthy meal prep and serve, hydration reminders (doesn't eat well-likes junk food) Make bed & check bed linens each day & change/launder as needed. Standby toileting assistance, help keep him clean and dry, standby assist with dressing & showers. Light housekeeping. May accompany on Dr appts if needed. Assist with exercises as per PT instructions. Retrieve mail.

Medications: Client takes his meds only 2x/day-once in AM before he eats and then at bedtime. CG can just ask if he has taken them.

ADL's:

Mobility: Client had a back injury so he doesn't stand often but can stand for about 5-10 min. He uses a walker or transport chair mostly. Client was observed to walk without assistance around his home.

Toileting: Standby assist only. CG should check bed linens each day & change/launder as needed.

Bathing/Dressing/Grooming: Client can shave himself but may ask for help. He ne

Qualifications:
Minimum of 1 year experience as an in-home Caregiver, CNA or related experiences with a loved one of family member
Ability to effectively communicate through written and verbal language
Valid Driver's License and adequate auto insurance
Consent to criminal, elder abuse, motor vehicle and sex offender background checks
Consent to drug and alcohol testing
TB test

Compensation: $16.00 - $17.00 per hour
