Responsibilities

Per Diem

Responsibilities:
  • Perform discharge- planning assessment, when indicated, within the required timeframe.
  • Foster a collaborative relationship with the medical staff and other members of the health care team to provide quality patient care and reduce LOS.
  • Calls/ conversations with physicians regarding discharge planning needs.
  • Bedside rounding with physician as often as possible.
  • Collaborates with members of the health care team to achieve a cohesive and comprehensive discharge plan for patients.
  • Develop discharge plans that are responsive to patient/ family concerns, health care status, available resources, age-specific problems and diagnosis specific issues.
  • Setting up discharge plan based on the assessment- dual plan if necessary. Finalize plan prior to end of acute illness and setting up day of potential discharge. This will require taking the lead and informing team/ MD/ MLP of time of discharge/ pick-up.
  • Provide appropriate referrals for patient/family to various resources.
  • Perform third party payer reviews as requested within the appropriate timeframe and document it.
Qualifications:
  • Graduate of an accredited school of social work; Graduate of Master's degree in social work required.
  • Two to three years' experience in health care related discipline (hospital, agency, etc.) and/ or previous discharge planning experience and/or community health care experience preferred. Must be extremely motivated, organized, flexible, and detail oriented; Excellent interpersonal skills (oral and written). Ability to respond to short deadlines. Effective problem solving abilities.
  • Must have as S.W. license in New York State; Screen certification preferred.
  • LMSW by the State of New York required. LCSW by the State of New York preferred.


Pay Range

USD $38.39 - USD $53.75 /Hr.
