FUNCTION:

The Registered Nurse (RN) will:

  • Provide direct support to people with disabilities for routine health care, conduct various assessments, address minor illnesses/injuries and ensure proper follow-up,

  • Make referrals to other medical professionals/facilities as appropriate,

  • Serve as a member of the Interdisciplinary Team and participate in the ISP planning and implementation processes as required,

  • Ensure adequate nursing personnel are always scheduled for the homes they oversee,

  • Provider supervisory oversight to Trained Medication Employees (TME) and ensure TMEs observation is completed as needed,

  • Complete nursing personnel timekeeping/related functions are completed in a timely fashion.

  • Provide general program support.

  • Complete monthly nursing notes for the person served and ensure upload in the appropriate mediums.

  • Conduct meetings with the Residential Coordinator (RC) and Qualified Intellectual Disability Professional (QIDP) and ensure Direct Support Professionals (DSP) are trained bi-annually and if there is any change in the person’s health, RC, QIDP and DSPs are informed and trained.

PRINCIPLE ACTIVITIES:

  • Be available, on-call as scheduled, for nursing consultation and/or treatment of minor illnesses or injuries and render nursing assistance in medical emergencies.
  • Develop rapport with each person receiving nursing care. Demonstrate respectful interactions at all times.
  • Participate in the initial admission, evaluation and treatment plan designed for each person admitted to this program.
  • Provide appropriate in-service training to staff in basic health care and hygiene skills.
  • Supervise and support DSP staff in the provision of medical support for the person.
  • Actively participate in activities related to medication administration:
  • Perform periodic medication administration review. Identify any errors and report accordingly.
  • Train, track Licensed Practical Nurses (LPN) and TMEs in Medication Administration training and all required documentation including the proper completion of supervisory medication passes.
  • Train and monitor TME trainings and document appropriately.
  • Regularly monitor the Medication Administration Record (MAR)
  • Participate as a member of the Interdisciplinary Team (IDT) providing general health counseling, review of medical procedures, medication and medical options with the person, their family, guardian and staff.
  • Help person and family coordinate all medical appointments.
  • Complete the following specific on-going tasks.
  • Make appropriate entries in nursing notes related to the care of the people supported.
  • Audit all nursing medical books routinely as designated.
  • Monitor adaptive equipment and update Health Passport accordingly.
  • Visit homes as deemed necessary per the Individual Service Plan (ISP) to monitor general medical status of people supported.
  • Develop and maintain Health Care Management Plans for all individual in the program and ensure that appropriate staff are involved in the process.
  • Ensure up-to-date documentation of all follow-up consults, such as, Health Passport, Nursing Assessment, Glasgow Scale, AIMS, Fall Risk Assessment, Braden Scale, Vaccinations, Annual Health Form, Self-Administration of Medication Assessment.
  • Maintain all appropriate medical documentation such as medical consultations, medical history, medication errors, etc.
  • Help coordinate all hospital discharge instructions.
  • Prepare medical summaries and attend Individual Service Plan (ISP) meetings as scheduled.
  • Complete identified assessments as needed at least annually for each person supported.
  • Communicate with the Clinical Nursing Manager regarding each person’s medical needs including routine, and well as, emergency treatments.
  • The RN should have working knowledge of community resources and a plan for accessing those resources.
  • Help coordinate all nursing-related information during program discharge or transfers. The nurse will also perform discharge-process education and other medically-based training to staff, and update the Health Passport.
  • Maintain up-to-date knowledge of Volunteers of America Chesapeake’s policies and procedures.
  • Be knowledgeable of the rules and regulations of the relevant service environments including Waiver and Res Hab and adhere to the program guiding principles.
  • Contribute meaningfully during internal audits and external monitoring visits including Evans Class Monitors, Quality Trust, HRLA, DOH, PCR, DDS, DDA in the District of Columbia.
  • Complete all necessary requirements related to MCIS-generated issues within thedesignated timeframes.
