Position: Nurse Practitioner

- Comprehensive Patient Care: Participate in facility rounds and provide gap care visits for patients in skilled nursing facilities and home settings, ensuring continuity of care and proactive management of patient needs.
- Clinical Oversight: Review abnormal vitals and labs, making recommendations or notifying attending physicians to facilitate timely interventions and optimize patient outcomes.
- Advanced Clinical Assessment: Independently perform age-appropriate history and physical exams for complex acute, critical, and chronically ill patients, demonstrating proficiency in clinical assessment and decision-making.
- Diagnostic and Therapeutic Management: Order and interpret diagnostic and therapeutic tests, prescribing appropriate treatments to address patients' healthcare needs effectively.
- Interdisciplinary Collaboration: Collaborate with multidisciplinary team members to facilitate patient transitions between healthcare settings, ensuring comprehensive and coordinated care for high-risk patients.
Qualifications:
NP Degree: Hold an NP degree from an accredited college or university.
Experience: Minimum of two years of related experience and/or training, demonstrating proficiency in providing care to high-risk patients with multiple chronic conditions.
