The Community Health Worker (CHW) is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers through an integrated approach to care management and community outreach. As a priority, activities will promote, maintain, and improve the health of patients and their family. CHW provides social support and informal counseling, advocates for individuals and community health needs.

Responsibilities
  • Assesses patient needs and offer culturally-competent resources.
  • Using active listening, assists patient with setting goal(s), provides follow-up communication to support patient reaching goal(s)
  • Communicates regularly with with clinical team on patient needs; serves as a liaison between patient and clinical providers when necessary
  • Identify new and changing community resources for patients
  • Perform basic brief screenings for ACEs, silent crisis situations, SUD and OUD
  • Documents all patient interactions in accordance with this position’s standard processes
    • Attends and participates in departmental meetings
    • Coordinates referrals to community partners, when appropriate, including procuring all required consents
    • Ensures appropriate boundaries maintained with patients and families
    • Collects additional data as needed for research, program evaluation, and program funder requirements
    • Maintains patient and visitor confidentiality
    • Maintains compliance with all company policies, procedures and standards of conduct
    • Complies with HIPAA privacy and security requirements to maintain confidentiality at all times

Performs other duties as assigned

Qualifications

Education:

  • High school diploma or GED/ equivalent (required)
  • Current participant in the Trager Paraprofessional Intern Program
  • Bachelor’s degree or Associate’s degree in public health, social work, or other related field (preferred)

Experience:

Two to three (2-3) years of experience working directly with people of various ages, backgrounds, ethnicities and life experiences preferred.
