We are seeking a compassionate and skilled Part-Time Clinical Case Manager to join our team at Encompass Health. In this role, you will play a crucial part in coordinating patient care from admission to post-discharge, ensuring quality and timely service for our patients. The position offers a range of benefits, including medical, dental, and vision plans, paid time off, tuition reimbursement, and more.

Responsibilities:

  • Work with interdisciplinary team to guide patient treatment plans
  • Coordinate post-discharge and contingency plans
  • Monitor patient experience and facilitate team conferences
  • Complete required documentation and maintain knowledge of regulations
  • Educate patients and families on rehabilitation and Case Manager role
  • Make appropriate referrals and ensure accuracy of discharge information
  • Participate in utilization review process and on-call schedule as needed

Qualifications:

  • Must hold an active clinical license and meet eligibility requirements for CCM® or ACM certification
  • Minimum of an Associate Degree in Nursing for RNs, bachelor's degree for other health care professionals
  • 2 years of rehabilitation experience preferred
