  • Assist in conducting focused studies and drug usage evaluations.
  • Contribute to the development of clinical guidelines and order sets for appropriate drug therapy.
  • Participate in decentralized care by collaborating with physician groups such as family practice, hospitalists, and critical care.
  • Intervene as appropriate to improve prescribing, administration, and monitoring practices related to medications for patients.
  • Represent the pharmacy on assigned committees or work groups.
  • Interpret physician medication orders and ensure accurate and timely medication dispensing.
  • Screen medication orders for potential interactions and supervise technicians in medication preparation and dispensing.
  • Make dose recommendations based on patient parameters.
  • Assist in training and orientation of new employees.
  • Ensure accurate accounting of all controlled substances and provide Medication Therapy Management Services as per regulations.
  • Perform utilization management reviews and serve as a drug information resource for other departments.
  • Conduct medication reconciliation, clinical interventions, identify adverse drug reactions, and refer patients to relevant clinics.
