Position Summary

RHD Connecticut is looking for a Part Time Registered Nurse to work in the North Branford area working 15-20 hours per week with flexibility. The Registered Nurse is responsible for overseeing all aspects of medical care for those individuals RHD provides services. The RN provides health and clinical oversight to individual with intellectual Disability and comorbid conditions. Performs assessments of person in their care to actual or potential health problems, participates in the development and implementation of the plan of care, and review and evaluate the effectives of the plan on a continuing basis. The RN provides clinical oversight / direction and delegates responsibility for performance of certain nursing task/responsibilities to trained non-licensed staff according to the standards identified by the Connecticut Nursing Board and the Department of Developmental Service regulation. Implements the plan of care as ordered / executed by the direction of the licensed physician, dentist, Physician Assistance, or APRN.

Essential Duties and Functions

  • Act as part of a multidisciplinary team designed to provide services from a holistic and values based perspective.
  • Responsible for the health care oversight of individuals residing in his/her current program assignment.
  • Help develop a plan of care in collaboration with assigned individuals and key multidisciplinary team members who support the individual in the assigned programs.
  • Monitors the overall healthcare, promotes health and wellness, as well as prevention of illness.
  • Nursing assessment, planning and implementation of individual plan of care.
  • Acts as a liaison to community medical providers to provide programmatic staff with education and support relating to the medical needs of the individual.
  • Provides timely nursing documentation as per CT DDS regulation.
  • Manage the ordering/delivery/storage/ administration of medications for the persons served in accordance with the specific program/service
  • Provide education to both persons served and staff regarding common health concerns. Provide training to staff as required/requested in order to best meet the needs of persons served.
  • Assess and respond to new/potential/ ongoing health issues in persons served.

Qualifications

  • A graduate of a school of professional Nursing (AD, hospital, or BSN); BSN preferred;
  • Maintains a valid current Connecticut Registered Nurse License.
  • Develop a work schedule, which allows for adequate oversight and supervision of assigned programs.
  • Perform job functions in a timely and professional manner.
  • Basic computer and internet literacy with the ability to utilize applications like emails, time recording and other programs to enter and retrieve data as necessary to perform the essential functions of the job.
  • Valid driver's license is required
  • Subject to employment screenings, including FBI clearance. if applicable

Lifting Requirements

  • Medium: exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds constantly to move objects.

RHD Connecticut provides residential community living arrangements, day services, and individual rehabilitative supports to people who have developmental disabilities in the West and South regions in Connecticut. Currently RHD CT operates 10 CLAs, Continuous Residential Support programs and supports individuals who reside in apartments in the community. RHD CT operates one day program in the South region of the State (Hamden).
