BASIC PURPOSE OF POSITION:

Under the general supervision of the DON assist in implementing and monitoring OBRA requirements related to RAI and Care Planning; and BrookHaven's Assessment Tools.

CUSTOMER/CLIENTS SERVED BY POSITION:

The residents, families, employees and physicians as well as corporate staff. May also interact with state surveyors and other governmental entities.

OUTCOMES WHEN JOB IS DONE WELL:

Resident Assessment/Care Planning requirements are met and are consistent with State and Federal mandates.

EDUCATION REQUIREMENTS:

Two or more years of experience in Long Term Care; working knowledge of the computer process.

SKILLS, LICENSE, CERTIFICATION AND/OR SPECIAL TRAINING REQUIREMENTS:

Holds a current nursing license. Coordinator - Must be an RN in Ohio. Nurse - Must be an LPN in Ohio. Meets state CEU requirements.

PHYSICAL, MENTAL AND SENSORY REQUIREMENTS:

Ability to sit, stand, bend, walk, reach to assist with residents well.

ESSENTIAL JOB DUTIES:

1. Has thorough knowledge of HCFA'S RAI system and BrookHaven Assessment Tools.

2. Completes and/or oversees completion of BrookHaven's Data Collection tools for Comprehensive Resident Assessment/MDS 3.0 on :

New Admissions

Annuals

Quarterlies

Significant Change of Condition

3. Completes computer generated MDS 3.0 and RAP/CATS Summary for total accuracy and acquires all appropriate signatures.

4. Reviews initial care plan, triggers report and RAP/CATS summary.

5. Responsible for scheduling and coordinating care conferences. Acquires appropriate signatures on care plan and RAP/CAT summary and assures their proper placement in resident's record. Assures continuity of information including assessments, care plans and audits.

6. Responsible for accurate tracking of the timely completion of all required assessments and care plans within federally mandated guidelines.

7. Completes/oversees completion of quarterly assessments, reviews computer generated quarterly MDS 3.0 updates care plans quarterly and acquires appropriate signatures.

8. Reviews change of condition forms and telephone orders daily to assure appropriateness of care plan updates, to identify significant change of condition, and monitor for proper notification of Physician and family

9. Assumes the role of RN Coordinator of assessment data; signs assessment/MDS 3.0 forms on time as RN Coordinator.

10. Develops/revises written procedures as related to RAI for the facility.

11. In-Servicing the interdisciplinary staff to new enhancements.

12. Participates in orientation of licensed, unlicensed staff, and the disciplines, as related to Federal, State, Corporate or facility requirements regarding RAI.

13. Provides routine in-services to educate staff on assessment practices of the facility.

14. Provides training to staff member designated by DON to perform duties in RN Assessment Coordinator's absence.

15. Responsible to perform routine Quality Assurance/QI monitoring and develop action plans related to RAI process and the Resident Care System.

16. Responsible to routinely perform Quality Assurance audit on implementation of Care Plan intervention. Other duties as assigned by supervisor.

17. Maintains system to ensure timely transmissions of MDS 3.0 to appropriate state agencies. Responsible to function as member of Nursing Administration team which includes participating in the on call rotation for facility.

NON-ESSENTIAL JOB FUNCTIONS:

1. Assesses prospective residents in conjunction with other member of the health care.

2. Participates in pre-admission planning and evaluating the appropriateness of referrals.

3. Serves as a liaison for all disciplines in planning resident programs/care.
