Job Summary

The Home Care Occupational Therapist provides therapy to patients based on patient need and medical direction. They also provide patient/caregiver education and arrange for adaptive equipment as needed.

Essential Functions

  • Perform evaluation procedures to assess the level of disability, create treatment plans, and make appropriate recommendations.
  • Provide treatment programs and education to restore maximal function to the patient, assess treatment effectiveness, and modify goals/treatment methods as needed.
  • Document evaluation and treatment of each patient in compliance with department standards.
  • Instruct and train other rehabilitation staff, medical personnel, and students regarding patient needs and rehabilitation services.
  • Participate in process improvement, patient satisfaction initiatives, program development, and marketing activities.
  • Demonstrate productivity as indicated by department standards.
  • Improve skill areas through continuing education, competencies, and developing new concepts of patient care.

Qualifications

  • Bachelor's Degree or equivalent from an accredited occupational therapy program (Required)
  • Master's Degree (Preferred)
  • 1 year or more of relevant experience within Home Care/VNA (Preferred)
  • Occupational Therapist, Registered (OTR) - NB-COT National Board for Certification in Occupational Therapy (Required)
  • Basic Life Support (BLS) - AHA American Heart Association or Basic Life Support (BLS) - ARC American Red Cross within 90 days of Hire (Required)
  • Driver's License - STATE_MI State of Michigan Home Care Business Lines Upon Hire (Required)
