Responsibilities/General Duties:
- Provides psychosocial support and other social work services to hospice patients and their families in the inpatient unit and residential settings.
- Provides and completes psychosocial assessments, discharge planning, direct casework, and counseling services to patients and families.
- Develop social work plan of care and discharge plans (beginning prior to or on the day of admission) when appropriate for patients admitted.
- Provides information to patient/family about applicable room and board charges, completing financial screening as applicable, prior to or on the day of admission.
- Participates in the preparation, evaluation, and execution of the interdisciplinary care plan to assure that the aims of continuity and comprehensiveness of care are accomplished.
- Works collaboratively as a member of the Interdisciplinary Team and participates in the Interdisciplinary Team meetings, staff meetings, and other meetings as requested.
- Prepares documentation for the clinical record per Hospice standards and ensures all documentation accurately represents all services provided.
- Demonstrates understanding and adherence to NASW Code of Ethics and related organizational policies, including confidentiality regarding patients, families, staff, and the organization.
- Maintains and updates knowledge of community resources, particularly long term care facilities (nursing homes and ALFs). Facilitates referral of patients/families to these resources as needed and desired.
- Develops strong, positive collaborative relationship with hospitals and long term care facilities (nursing homes and ALFs). ? Participates in evening/weekend coverage responsibilities.
- Maintains quality and productivity standards.
- Applies knowledge of emotional, social, and spiritual needs and responses to death and grief including factors that indicate complicated grief and high survivor risk.
- Participates in providing support to staff and volunteers in coping with job-related stress.
- Participates in agency and community programs, including supervision, to enhance learning and performance and remains current in topics/research in hospice and grief.
- Participates in the orientation of volunteers and staff and supervises field placement students when applicable.?
- Facilitates communication with outside agencies including Region 10 and Social Services.
- Implements and plans for placement and discharges from facilities, hospital, and hospice as well as level of care changes.
- Provides Hospice House tours for families as needed.
- Takes primary responsibility within the IDG for Discharge Planning, Advance Directives, Bereavement referrals, APS referrals, funeral planning, volunteer referrals, correct demographics in the EMR, current insurance information and paperwork related to discharges and level of care changes and placements. ?
- Is flexible regarding work schedule depending on patient/family need; may require some evenings/weekends.
- Completes other assignments and tasks as requested.
- Prefer a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education or requires a baccalaureate/Bachelor's degree in social work, psychology or sociology and is supervised by an MSW.
- Current state licensure (LCSW) preferred.
- Two years of social work experience in a social service setting with at least one of the two years being in a healthcare setting required.
- Experience in inpatient discharge planning and knowledge of long term care facilities preferred.
- Knowledge of the process of death, dying and bereavement and understanding of psychosocial factors operative in terminally ill patients and families required. Clinical experience with individuals, groups and families.
- Knowledge of the principles and practices of social work, counseling techniques, hospice philosophy and group dynamics.
- Skill in counseling, interviewing and communicating difficult news.
- Knowledge of resource/support agencies and methods of accessing services.
- Intermediate proficiency in Microsoft Word software and EMR system.
- Excellent interpersonal and communication skills.
- Must be flexible and able to function in stressful situations.
- The ability to work as a team member.
- Ability to work with and maintain confidentiality of patient data.
