Key Responsibilities:

• Manage the Minimum Data Set (MDS) assessments for personalized resident care plans
• Ensure compliance and accuracy in assessments
• Monitor Case Mix Index (CMI) scores for reimbursement
• Collaborate with interdisciplinary teams for accurate data collection
• Provide ongoing education to facility staff and leaders
• Track, trend, and analyze assessment data for compliance and optimization

Qualifications:

• Valid state RN license preferred
• Direct care and/or MDS Coordinator experience required
• Proficient in state and federal regulations governing the MDS and billing process
• Deadline driven, exceptionally detail-oriented individual
• Excellent written and verbal communication and interpersonal abilities
