Physical Therapist

Job Summary: We are looking for a committed and compassionate Physical Therapist to join our team. As a Physical Therapist in our home health care agency, you will play a crucial role in the recovery and rehabilitation of our patients. You will work closely with patients to restore their mobility, improve their quality of life, and help them achieve their personal best. Your expertise will be essential in promoting health, preventing injury, diagnosing conditions, and providing effective treatment and rehabilitation.
Key Responsibilities:
- Patient Assessment: Conduct thorough assessments of patients' physical conditions, functional abilities, and mobility. Develop individualized treatment plans based on these assessments.
- Treatment and Intervention: Implement evidence-based treatment plans to address patients' specific needs. Utilize a variety of techniques, including exercises, manual therapy, and specialized equipment.
- Rehabilitation: Guide patients through the rehabilitation process, helping them regain strength, flexibility, and independence. Monitor progress and adjust treatment plans as necessary.
- Education and Support: Educate patients and their families about their conditions, treatment plans, and home exercise programs. Provide ongoing support and encouragement to ensure compliance and optimal outcomes.
- Collaboration: Work closely with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive and coordinated care.
- Documentation: Maintain accurate and up-to-date patient records, including assessments, treatment plans, progress notes, and discharge summaries.
Qualifications:
- Education: Doctorate in Physical Therapy (DPT) from an accredited program.
- Licensure: Current state license to practice as a Physical Therapist.
- Experience: Previous experience in home health care is preferred but not required. New graduates are welcome to apply.
- Skills: Strong clinical skills, excellent communication and interpersonal abilities, and a compassionate and patient-centered approach to care.
- Certifications: CPR certification and any other relevant certifications.
