Job Description

Under the direction of the UR Administrative Supervisor, this position is responsible to assist patients and department staff in the discharge coordination from inpatient, ER, or skilled nursing facility setting by ensuring authorizations are generated and issued timely. Also responsible for scheduling appointments for those patients requiring follow up after hospitalization, ER visit (if seen by BFMC/CCPN hospitalists), and skilled nursing facilities.. The Discharge Coordinator is responsible to scan discharge plans, orders, member letters and records accurately and timely into NextGen EHR system.

  • Ability to verify eligibility, benefits when necessary.
  • Responsible to accurately generate authorizations to appropriate providers with accurate ICD-10, CPT, or HCPC codes along with provider specialty in accordance with post hospital discharge orders, departmental and regulatory timeliness standards.
  • Coordinate with case managers the ordering and delivering of DME to facilitate the patients leaving the facilities in a timely manner.
  • Schedule appointments for patients discharged from the hospital, emergency room (when discharged by BFMC/CCPN hospitalists) or skilled nursing setting.
  • Communicate in a positive, helpful, polite, respectful and professional manner with fellow staff and outside vendors.
  • Maintain patient confidentiality within HIPAA regulations.
  • Inform supervisor when problems arise, delay in completing assignments, or work falls behind.
  • Responsible for adhering to departmental, CMS and health plan requirements in relation to generating authorizations, and notification of determinations within regulatory regulatoryrequirements.
  • Maintain filing and documenting into departmental systems in a timely manner, including scanning into NextGen.
  • Receive and process incoming phone calls, fax and emails and respond in a timely manner.
  • Mail authorizations and appointment letters on a daily basis as required.
  • Scanning of discharge summary plans, post hospitalization orders, member correspondences, and hospital records accurately and in a timely manner into NextGen EHR system.

  • Thorough documentation in NextGen of actions taken to fulfill discharge plan orders and provider office and patient communications.
  • Timely follow through with completing assigned tasks within departmental and regulatory standards.
  • May be required to work weekends and/or overtime as assigned.
  • Cultural and Linguistics training required annually.
  • Other duties as assigned
  • Scan in charts and other medical records into NextGen and assigning of discharges to staff.
Requirements
  • High school diploma or GED certification, required.
  • One year minimum working in a medical office environment,required.
  • Previous experience with authorizations, preferred.
  • Working knowledge of computersoftware.
  • Medical terminology training or experience, preferred.
  • Experience with CPT, HCPC, and ICD-10 coding, required.
  • Good phone etiquette.
  • Excellent organizational and communication skills.
  • Typing certificate of 40 wpm, required.
  • Proficient in computers skills including but not limited to Excel and Word required.


The pay range for this position at commencement of employment is expected to be between $20.44 and $28.05 however, base pay offered may vary depending on multiple individualized factors, including market location, job-related knowledge, skills, and experience. The total compensation package for this position may also include other elements, including a sign-on bonus, restricted stock units, and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered. Details of participation in these benefit plans will be provided if an employee receives an offer of employment.
