Day in the Life

The Medicaid Eligibility Case Manager ensures residents in the Kosair for Kids Complex Care Center are financially eligible for admission and continued stay in the skilled nursing facility. Working under the Director of Medical Social Services and with the Medical Social Services team, this role guides parents and guardians on Medicaid and Medicare eligibility, Supplemental Security Income, Nursing Home Level of Care, and the PASRR process. The Case Manager ensures compliance with federal, state, and local regulations, supporting an efficient agency revenue cycle.

Medicaid Eligibility Case Manager Responsibilities:

  • Conduct eligibility screenings and recertification's for resident financial data
  • Assist parents and guardians with Medicaid and/or Medicare applications
  • Act as a representative payee for residents when necessary
  • Collaborate with the finance team to ensure a smooth revenue cycle
  • Serve as a liaison between residents' responsible parties and governmental agencies
  • Maintain accurate and timely documentation for eligibility determination
  • Establish and maintain financial records
  • Treat all stakeholders with respect and confidentiality, ensuring protection from abuse and neglect
  • Promote a culture of diversity, inclusion, and non-violence
  • Exemplify the Core Values of Compassion, Wellness, Excellence, and Empowerment
  • Perform other duties as assigned
Medicaid Eligibility Case Manager Skills and Qualifications:
  • Function independently with flexibility, integrity, professionalism, and compassion
  • Strong analytical and communication skills
  • Proficient with Microsoft Office Suite
  • High School Diploma or equivalent required
  • Minimum one year of experience in Medicaid, Medicare, insurance, benefits eligibility, reimbursement, or case management, Long-term or acute care experience preferred
  • Knowledge of nursing home admission eligibility criteria, Medicaid and Medicare regulations, and Social Security program requirements
  • Current CPR/Basic Life Support certification required (provided if necessary)
