About the Role:

We are looking for a compassionate and empathetic Hospice RN Case Manager to join our team. As a key member of our hospice care team, you will be responsible for providing direct patient care services, supporting the patient's family, and collaborating with an interdisciplinary group to ensure comfort, peace, and dignity for our patients.

Key Responsibilities:

  • Report directly to the Administrator, Executive Director, or Patient Care Manager
  • Act as the primary liaison between physician, patient/family, and hospice team
  • Empower patient's caregivers through education and support
  • Evaluate patient and family needs and assist in the preparation and execution of the plan of care
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs

Requirements:

  • RN license in the state of requested employment (or eligibility to obtain)
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred
  • Current driver's license and automobile insurance
  • Current CPR certification
