Nurse Practitioner Residency Program at Esperanza Health Center

Esperanza Health Center's Nurse Practitioner Residency Program is a full-time, 15-month, salaried position that provides a structured, supportive, and effective training environment for new graduate nurse practitioners. The program includes precepted primary care sessions, mentored independent sessions, specialty rotations, a quality improvement project, and weekly didactic sessions.

We are excited to train the next generation of competent and compassionate nurse practitioners who are passionate about working in an urban setting and providing holistic care that is compelled by the love of God in Christ Jesus.

APPLICANT ELIGIBILITY

Eligible candidates will:
  1. possess a sincere interest in primary care;
  2. have graduated from an accredited Family Nurse Practitioner program in the past 24 months;
  3. possess an active, unencumbered nursing license in the state of Pennsylvania;
  4. be board-certified as a Family Nurse Practitioner through either the AANP or ANCC BEFORE June 1st, 2025;
  5. be proficient in English and Spanish. Immersive language training is available with a stipend prior to employment if needed;
  6. demonstrate commitment to and ability to provide a holistic model of care that addresses patients' physical, emotional, spiritual and social health that is compelled by the love of God in Christ Jesus.


GOALS

By the end of this residency program, the resident will be fully equipped to:
  • Effectively care for complex primary care patients in an urban FQHC setting
  • Demonstrate clinical leadership skills in communication, collaboration, critical thinking, and integrity using an integrated team model
  • Understand how inequities in social drivers of health affect the health and wellbeing of individuals and communities
  • Integrate cultural awareness and sensitivity into every aspect of clinical practice
  • Be knowledgeable in quality improvement, population health, and integrated health systems
  • Provide effective spiritual intervention with patients, sharing hope through Jesus Christ, according to the leading of the Holy Spirit, motivated always by love and humility
  • Practice to the fullest extent of the nurse practitioner license
DUTIES AND RESPONSIBILITIES

Essentials
  • In a precepted clinical session, the resident will be seeing patients off of their own schedule at a rate predetermined by the ramp-up schedule. The resident will provide a thorough, concise, and organized report to their assigned preceptor, including an assessment and plan in line with current clinical guidelines. The resident will communicate the final plan (as agreed upon by the preceptor) to the patient and complete the visit.
  • In a mentored clinical session, the resident will be seeing patients off of their own schedule at a rate pre-determined by the ramp-up schedule. They will be working in close physical proximity to a "mentor", who will be an experienced clinician in the adjacent hub and who will be available for consultation as needed.
  • The resident will be responsible for all appropriate documentation and billing for patient visits. They may route notes to the preceptor as desired for co-sign and additional feedback. This is an encouraged practice, particularly at the beginning of residency.
  • During specialty clinics, the resident will shadow the specialist in an engaged, yet not burdensome manner. Specialties include women's health, opioid use disorder, HIV, lifestyle medicine, and sports medicine.
  • During didactic sessions, the resident will effectively and efficiently participate in the educational topic assigned for that day which will include case presentations.
  • Occasionally, the resident will lead the didactic time in a flipped classroom format.
  • The resident will be responsible for the completion of a Quality Improvement project by the end of the program. Topics will be approved by the Residency Director and the Director for Medical Education.
  • Beginning in month 6 of the program, all NP residents will be expected to take after-hours call by phone at the same rate as any other full-time clinician. This is typically 1-5 nights per month, including weekends.
  • Beginning in month 6 of the program, all NP residents will be required to work Saturday morning clinic (9am-1pm) at the same rate as any other full-time clinician. This is approximately once per month.
  • The resident will participate in team-based care for patients, involving behavioral health, social services, care management, and other clinical teams.
  • The resident will learn to provide holistic care to each patient that is consistent with Esperanza's mission statement, primarily that it is compelled by the love of God in Jesus Christ. This could involve, but is not limited to, praying with patients, referring to local churches, and collaborating with staff chaplains.

To carry out its mission, it is of critical importance for Esperanza Health Center (EHC) to keep our patients, families, and workforce safe and healthy and to support the health of our global community. In keeping with this, EHC has mandated all workforce members on-site at any EHC location for any portion of their time be vaccinated for COVID-19. This also includes contractors and EHC volunteers.
