- Educate patients and families on disease progression, symptom management strategies, and the overall care process.
- Coordinate and administer medical treatments to ensure patient comfort and symptom control.
- Approach all duties with a heart for hospice and palliative care, delivering services with empathy, dignity, and respect.
- Must be a fully licensed Nurse Practitioner with minimum 1 year Home Care experience
- Experience in palliative care is a plus but not required.
