Nurse Practitioner | Physician Assistant

A healthcare services company in New York City is currently seeking a new, licensed Nurse Practitioner (NP) / Physician Assistant (PA) to join their team in New York.
Responsibilities:
The Nurse Practitioner (NP) / Physician Assistant (PA) will:
- Review and make recommendations on consumer assessments for long term care services and supports.
- Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments.
- Assess cases presented and utilizes clinical competencies to review and validate assessment outcomes.
- Work collaboratively to monitor and address clinical quality and gaps in care.
- Provide clinical and educational assistance to support project initiatives.
- Demonstrate communication skills necessary to interpret age-specific responses.
- Participate in performance, improvement, and quality assurance activities and related meetings.
- Provide clinical and educational support to meet program goals.
- Perform other duties as may be assigned by Management.
- 2+ years of relevant work experience
- Master's Degree from a Nurse Practitioner or Physician Assistant Program
- NYS Nurse Practitioner (NP) and/or Physician Assistant (PA) license
- Board Certified Nurse Practitioner (NP) and/or Physician Assistant (PA)
- Computer savvy
- Board Certified in Internal Medicine, Adult Health, Community Health, and/or Family Practice
- Bilingual (English and Cantonese, Mandarin, Bengali, Haitian-Creole, Russian, or Spanish)
- Experience working with the Adult / Geriatric populations.
- Home Care background
- Experience working with individuals with Physical and/or Developmental Disabilities
- Knowledge of Long-Term Care services & programs
