Pharmacist - Farmingdale, NY

Position: LTC (Long-Term Care) Pharmacist

Schedule: Mixed

• 2 days: 3:30 PM - 12:00 Midnight

• 2 days: 1:30 PM - 10:00 PM

• Sunday: 7:00 AM - 7:00 PM

Job Summary:

We are seeking a dedicated and skilled Long-Term Care ( LTC ) Pharmacist to join our team in Farmingdale, NY. The ideal candidate will have experience in LTC pharmacy , though we are open to considering those with retail experience who are eager to transition into this specialized area. The candidate should be comfortable with the varied shift timings as listed above.

Key Responsibilities:

• Accurately dispense medications for long-term care facilities, ensuring compliance with all legal and regulatory standards.

• Review and interpret physician orders, ensuring the appropriate medication is provided to the patient.

• Provide medication counselling and information to healthcare providers and patients.

• Monitor patient outcomes and collaborate with healthcare teams to optimize therapeutic efficacy.

• Maintain accurate records and documentation for all pharmacy activities.

• Ensure a smooth workflow during each shift and provide leadership when necessary.

• Adapt to the varying schedule with professionalism and efficiency.

Qualifications:

• A valid and active Pharmacist license in the state of New York.

• Experience in LTC pharmacy is highly preferred; retail pharmacy experience will be considered.

• LTC & QS/1 experience a plus.

• Strong attention to detail and excellent communication skills.

• Ability to work the mixed schedule as outlined.
