Pharmacy Intern/Inpatient fill orders for medications, monitor patient medication therapies, and provide drug information under the direct supervision of a Pharmacist. Tasks are subject to sequential or final verification by a registered Pharmacist.
Roles and Responsibilities:
- Interprets medication orders (verbal and written), transcribes to computerized patient profiles accurately, and maintains accurate, complete patient medication profiles; prepares and dispenses medications, including sterile IV preparations, using proper techniques.
- Issues controlled substances to patient care areas, maintains records as required by law, and assists with pharmacokinetic and anticoagulation consult services per organizational protocol.
- Monitors medication therapy regimens for contraindications, drug-drug, drug-food, drug-laboratory interactions, allergies, and appropriateness of drug, dose, and therapeutic duplication.
- Reviews and interprets culture and susceptibility data for antibiotic appropriateness and recommends changes.
- Detects and reports suspected adverse drug events and medication errors accurately and timely; attends ABC alert codes.
- Provides clinical consultation and clarification to nursing and provider staff; recommends evidence-based medication therapy regimens and monitoring plans; suggests appropriate cost-effective therapeutic alternatives to medical staff; and is flexible in assuming other appropriate responsibilities.
- Education - Completion of the first professional year of study in an Accreditation Council for Pharmacy Education (ACPE) program, or a graduate from a non-accredited program that passed the NAPLEX and is approved by the board office, eligible to apply for a limited (intern) permit. This permit will authorize practice as a pharmacist under the immediate personal supervision of a licensed pharmacist in New York State.
- Experience - Health-system pharmacy experience preferred, but not required.
- Licensure - Must hold a Limited (Intern) Permit from the State of New York. A pharmacy intern shall have 6 months to obtain NY State Pharmacist license and registration. If a pharmacy intern is unable to obtain NY State Pharmacist license and registration within the initial 6 months of hire as a graduate intern, then the Pharmacy department reserves the right to re-evaluate ongoing employment.
