Works in collaboration with a physician and other palliative care team members in an active practice to deliver hospice and palliative care and related health care services to patients and families, primarily in a hospital setting with occasional home visits.
Position Details
- Work Locations: Shands and North Florida Hospitals; occasional home health visits
- Days/Hours: Monday - Friday 830am-5:pm; usually one weekend a month flexible team scheduling
- Compensation: $107-120K base salary; full benefits package including 23 days PTO annual
- Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and education for both patients and family.
- Assists with diagnosis, treatment and management of acute and chronic health conditions.
- Orders, performs and interprets laboratory and radiology tests within scope of professional practice.
- Writes orders pertaining to informed consents, diagnostic procedures and treatments.
- Prescribes medications and other treatments.
- Administers medications as prescribed.
- Consults with palliative care physician or designee as needed, informs attending physician of services provided and collaborates with other physicians as needed.
- Facilitates clarification of patient and family goals of care.
- Prepares and maintains accurate patient records, charts and documents to support sound medical practice and reimbursement of services provided.
- Executes physician's orders in accordance with legal, ethical, and organizational standards.
- Complies with applicable laws and regulations.
- Initiates reimbursement for services rendered on a timely basis.
- Ensures provision of informed consent for CPR and DNR.
- Recommends medications as required to treat specific populations.
- Adheres to the practice of confidentiality regarding patients, families, staff and the organization.
- Provides educational sessions to advance palliative medicine among internal staff, medical community and community as a whole.
- Participates in on-call coverage and assists in establishing schedule.
- Participates in regular interdisciplinary team meetings to review active cases.
- Graduate, advanced education with Master's degree
- Minimum of two (2) years nursing experience.
- Experience in hospice palliative care preferred
- Valid State of Florida Driver's license with driving record acceptable to Community Hospice insurance carrier.
- Advanced Practice Nurse licensed in the State of Florida
- DEA license for prescribing controlled substances
