Summary:
The pharmacist, in this position, is responsible for providing comprehensive pharmacy medication management in the emergency department (ED). As a member of the multidisciplinary healthcare team, the clinical pharmacist promotes an environment of safe, rational, effective and efficient medication utilization while providing services to meet the needs of the patient. In addition, the ED pharmacist performs other pharmacy services as appropriate.
RESPONSIBILITIES
• Reviews orders for appropriateness and completes final verification.
• Assesses the effectiveness, safety and affordability of patient medications. Works collaboratively with the ED healthcare team to develop and optimize patient medication therapy.
• Provides drug information on medication use, dosage, side effects and composition to physicians, nurses, and patients/families.
• Monitors inpatient and outpatient profiles for drug interactions, food-drug interactions, and duplication of therapy.
• Participates in transitions of care and assists with issues involving patient access to medications.
• Participates in quality improvement activities including but not limited to floor inspections, medication use evaluations, medication histories capture and documentation of interventions.
• Provides knowledge of best pharmacotherapy practices for emergency pediatric medicine and how the ED area integrates with other service lines.
• Demonstrates awareness of the industry, including regulatory requirements, evolving customer demands, and factors that differentiate the organization in the market.
• Supports contributions to pharmacy and medical literature and participates in research where applicable.
• Maintains knowledge in area of expertise.
• Provides education to pharmacists, pharmacy residents, nurses and providers. Precepts pharmacy students, residents and fellows.
• Facilitates the compounding, labeling and packaging of medications and pharmaceuticals
• Responds to code and/or trauma pages within the ED.
• Assist in maintaining the department drug inventory at acceptable levels and ensures security of narcotics.
• Participates in the development, implementation and ongoing review of Epic medication orders and medication order set builds.
• Attends hospital committee meetings as needed.
• Participates in continuing education programs, in-services, and workshops to maintain current knowledge and enhance professional growth.
• Performs other duties and special projects as required.
This will be a Full Time, nights position in the Emergency Department.
WORK EXPERIENCE
• At least 3 years previous pediatric hospital experience- Required
• At least 3 years previous pediatric experience in critical care or emergency medicine- Preferred
EDUCATION
• Four-year Bachelor's degree or equivalent experience Degree (B.S. or Pharm.D) from an accredited school of pharmacy- Required
• Graduate, professional work, advanced degree, or equivalent experience Doctor of Pharmacy - Preferred
• Completion of PGY-1 ASHP-accredited Pharmacy Residency (pediatric focused) - Required
• Completion of PGY-2 ASHP-accredited Pharmacy Residency in Critical care or Pediatric Emergency Medicine - Preferred
• 20-hour ACPE accredited IV training course, except in Retail / First Pharmacy areas. Required within 90 days- Required
LICENSES AND CERTIFICATIONS
• Current Texas Pharmacist license- Required
• Basic Life Support for Healthcare Providers as required by CP 1.20 Life Support Course Requirements- 90 Days - Required
• PALS Certification for all emergency care and critical care clinical pharmacists- 90 Days Required
• ACLS Certification for all emergency care and critical care clinical pharmacists- 90 Days Required
• Board of Pharmacy Specialties Certification- Preferred
JOB PROFILE
• Provides comprehensive pharmacy medication management review of ED medication orders including during medical emergencies, code resuscitation efforts, and high-risk procedures.
• Completes comprehensive medication order verification reviews to include but not limited to appropriate indication, dosing, allergy assessment, organ function, IV to PO conversion, antimicrobial stewardship and drug-drug and drug-disease interactions.
• Requires depth within a specialized, critical discipline and/or breadth of substantive professional knowledge that crosses disciplines within the professional field.
• Interprets internal/external business challenges and recommends best practices to improve products, processes or services.
