Description

Position Title: Registered Dietitian
Department: PACE
Supervisor: Nutrition Manager
Status / Hours per week: Exempt / 35 hours

Primary Function:
To provide nutrition services to Upham's PACE participants including nutrition assessment, education, and counseling.

Duties & Responsibilities:
• Perform initial and periodic nutrition assessments and screenings.
• Work with interdisciplinary teams to develop care plans that best support the unique social, mental, and health needs of participants.
• Provide nutrition education, counseling, and medical nutrition therapy to participants and caregivers in-home, at the PACE center, and via virtual platforms as appropriate.
• Coordinate and oversee home-delivered and congregate meal programs.
• Participate in the development and implementation of group nutrition activities (ex: cooking demos, group education, farmer's market tours, etc.).
• Take participant weights and other vital signs as needed with demonstrated competency.
• Work closely with Social Work department to develop food access plans (ex: online grocery ordering, SNAP, food pantry referrals).
• Conduct staff education in-services on food and nutrition topics.
• Attend daily multidisciplinary meetings (in-person and/or virtual).
• Document in the electronic health record.
• Participate in departmental quality improvement activities.
• Provide coverage for nutrition department as needed.

Qualifications

Experience & Qualifications:

Experience:
• Two (2) years of experience in clinical, outpatient, or community nutrition.
• Experience or strong interest in working with older adults.
• Comfort in working with a multicultural and multi-lingual population.

Qualifications:
• Registered Dietitian or Registered Dietitian Nutritionist with the Commission on Dietetic Registration.
• Active licensure with the Massachusetts Board of Registration of Dietitians and Nutritionists.
• ServSafe Food Safety Manager or equivalent preferred.
• Multi-lingual preferred (Spanish, Portuguese, Cape Verdean, or Haitian-Creole).
• Maintenance of continuing education requirements.
• Ability to travel to participant homes and between multiple PACE center locations as needed.
• Ability to work both independently and within an interdisciplinary team environment.
• Good communication, leadership, time management, and decision-making skills.

Supervisory Responsibility:
No formal supervisory responsibilities

Define Access Level to PHI: Level 4: Authorized to access full health information. UCC position and job responsibilities involve the provision of patient care and working as a clinical team member. Staff in this category level, although allowed full access, should only access the necessary information for each respective treatment encounter/circumstance.
