The Community Outreach RN Coordinator is responsible for coordinating and implementing Hackensack Meridian Health's Community Outreach Wellness Plan to address identified community health needs and in support of Hackensack Meridian Health's strategic imperatives and centers of excellence. Implements and evaluates health education programs, and coordinates the provision of health education services.

This position will require traveling in the community at least 50% of the time. A flexible schedule is required, schedule/hours can change weekly.
