WHAT YOU WILL DO:

?The Safe at Home (SAH) Case Manager is a non-exempt position responsible for leading facilitation of team services in the Safe at Home program model. This position works in collaboration with the Family Advocate and Counselor to build the natural supports of the family, including providing linkage to formal (community providers) and informal (family, friends, neighbors, church, school, etc.) resources to clients involved with Florida Child Protective Services to ensure child safety needs are met in conjunction with the provision of family strengthening interventions.?

  • Conduct home visits with parents, caretakers, at-risk children, their families, and extended support networks
  • Provide information, referral and follow-up for appropriate ancillary community counseling, psychiatric services, financial, employment, parenting, housing, and other services to meet family needs
  • Create individualized plans and needs assessments, and coordinate/conduct Family Team Meetings with the family (and formal and informal supports when these extended supports are available) to address child safety and the issues which have contributed to the family's need for interventions from the program, by the required contractual timeframes
  • Create and maintain collaborative relationships with peers, stakeholders, case management, and providers with a mutual goal of child safety and family preservation
  • Help families discover and gain access to community resources that can assist in meeting their needs
  • Keep accurate record of all activities, including the documentation of all phone calls and linkage with community supports, as well as home visits by required contractual timeframes
  • Knowledge of Safe at Home performance outcome measures and contractual requirements
  • Provide peer support and mentorship to volunteers, interns, peers, etc.
  • Perform all other duties as assigned

WHAT YOU WILL NEED:
  • Bachelor's degree from an accredited college or university in social work or related areas of study and one (1) year experience working with children and/or families preferred?
    • Or an Associate's degree from an accredited college or 60+ college credits from an accredited college or university AND one of the following: Two (2) years of professional work experience or two (2) years of full-time social work or human services experience, required?
  • Must complete the Child Welfare Case Manager Certification Training within 12 months of employment.
  • Must have a valid drivers' license with current auto insurance coverage?.
