Develops and collects data as outlined in the grant and of importance in the operations.
  • MAINTAINS EFFECTIVE COMMUNCIATION AND TEAMWORK STYLE WITH COWORKERS, OTHER DEPARTMENTS, OTHER HEALTH CARE PROVIDERS AND THE COMMUNITY.* Will assist in the development and implementation of educational programs as needed and indicated in the grant.* Actively participate in communication, attendance in required meetings, and learning collaborative sessions with funder and other external partners.* Actively maintains a professional relationship with open communication with providers, units, clinics Actively maintains open communication & demonstrates a cooperative, teamwork style with patients, families, other staff members, other healthcare workers, medical staff and community Demonstrates organizational abilities through correct prioritization of duties, efficient and appropriate usage of time, and effective utilization of support personnel.
  • Required Documentation for EMR and Grant. Ability to understand data and provide analysis of the data Collection and reporting of data to inform process evaluation of the development, implementation, and functioning of the project Maintains computer proficiency and audit reporting of SAMHSA-SOR required data* Ensures accurate documentation* Assist with documentation in EMR to support quality initiatives* Assist with audits for medical necessity documents as requested* Oversees and ensures that data is entered timely and correct in the identified grant data base

Responsible for coordinating day-to-day clinical services, operations, data collection, and leadership of the recovery navigators.
