About Us: Lincoln Meadows Care Center is committed to providing outstanding care in a supportive and nurturing environment. We are seeking a dedicated part-time Physical Therapist to join our team and help our residents achieve their rehabilitation goals.

Key Responsibilities:
  • Evaluate residents' physical conditions and develop individualized treatment plans
  • Provide hands-on therapy to improve mobility, strength, and functional abilities
  • Monitor and document progress, adjusting treatment plans as needed
  • Educate residents and their families on therapeutic exercises and techniques
  • Collaborate with other healthcare professionals to ensure comprehensive care
Qualifications:
  • Valid state license as a Physical Therapist
  • Strong clinical and interpersonal skills
  • Compassionate and patient-centered approach
  • Ability to work independently and as part of a team
  • Previous experience in a healthcare setting preferred
