Hospice LPN Job Details Level Experienced Job Location Main Office - Newton, MA Hospice LPN Named by The Boston Globe for the second year in a row as one of theTop Places to Work in Massachusetts,our community-based, mission-driven hospice and palliative care organization seeks experienced, talented, and compassionate hospice Licensed Practical Nurses to join our team. Travel is required for this position. Care is provided in patient’s private homes and residential facilities throughout Greater Boston and parts of Metrowest. Job Responsibilities: The Hospice LPN provides quality nursing care for all hospice patients following the established plan of care. The ideal candidate understands the importance of providing quality service and compassionate end-of-life care for clients. Is professional, reliable, organized, and demonstrates effective communication skills. Must have demonstrated skills in patient/family evaluations, pain management, symptom control, and communication. Education, Skills, and Experience: Licensed Practical Nurse (MA license) A minimum of two years prior experience in community health, long term care facility, oncology, or a related field preferred. Ability to be a team player and work harmoniously with patients, families, co-workers, and management. Providing care consistent with written instructions. Providing assessments of the impact of the terminal diagnosis on the patient’s physical, functional and environmental needs and activities of daily living. Providing on-going evaluation of patient/family response to care and informing the Nursing Care Coordinator and other personnel of changes in the patient’s needs and outcomes of interventions. Preparing documentation that demonstrates adherence to the patient’s individualized plan of care and progress towards established goals. Assessing for patients’ safety. Providing personal care including assistance with daily hygiene, ambulation, exercise, toileting, nutritional support, dressing changes and other care as designated by the Nursing Care Coordinator. Assisting with medications that are ordinarily self-administered. Assisting patients with household activities.
