Role Description
We are currently seeking a dedicated and skilled Physical Therapist to join our team. As a Physical Therapist, you will work closely with patients to help them recover from injuries, manage chronic conditions, and improve their overall physical function. Your expertise in movement and rehabilitation will play a vital role in restoring patients' mobility, reducing pain, and enhancing their quality of life.
Responsibilities
- Perform thorough evaluations and assessments to determine patients' physical limitations, functional abilities, and rehabilitation needs.
- Develop personalized treatment plans based on patients' individual goals, medical history, and current condition.
- Monitor patients' progress regularly and make necessary adjustments to treatment plans as required.
- Collaborate with other healthcare professionals, including physicians, occupational therapists, and nurses, to ensure comprehensive patient care.
- Maintain accurate and detailed documentation of evaluations, treatment sessions, progress notes, and outcomes.
- The physical therapist is responsible for overseeing and supervising physical therapists assistants.
- Experience in long-term and geriatric care is preferred but not limited to.
- Must handle compliance with confidentiality, and understand HIPPA and Federal Residents Rights.
- Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
- Stay updated with the latest research, advances, and best practices in the field of physical therapy.
Requirements
- Bachelor's or Master's degree in Physical Therapy from an accredited program.
- Valid state licensure as a Physical Therapist.
- Clinical experience in a variety of settings, such as hospitals, clinics, or rehabilitation centers, is preferred.
- Net Health software experience is preferred but not required.
- Excellent communication and interpersonal skills to effectively interact with patients, caregivers, and healthcare professionals.
- Empathy and compassion for patients undergoing physical therapy.
