Position Summary:

Working within the Harm Reduction Paradigm, the APN will support the mission and goals of The Night Ministry (TNM) by providing health care, education, and prevention services across various outreach settings, including the Health Outreach Bus, Street Medicine, CTA, and youth programs. This role involves offering hospitality, health education and assessments, counseling, medical treatment, and referrals for further appropriate care to individuals experiencing homelessness and those without access to adequate health care, while improving and expanding TNM's health care capacity.

Key Responsibilities:
  • Enter data accurately and promptly in systems such as Electronic Health Records (EHR) and Efforts to Outcomes (ETO).
  • Perform basic physical assessments and treat acute and chronic conditions across all age groups.
  • Administer Medication-Assisted Treatment for opioid use and support clients in substance use disorder treatments.
  • Collaborate with staff and volunteers to plan and deliver outreach services.
  • Provide and manage referrals for health care, including maintaining an updated referral list.
  • Document health-related activities and services as required by funding or accreditation standards.
  • Oversee supply storage and inventory management.
  • Attend required internal and external training sessions.
  • Travel to and provide services at various outreach locations, such as shelters.
  • Participate in department, agency, and inter-agency meetings.
  • Carry a cell phone per policy, with eligibility for partial reimbursement.
  • Perform other duties as assigned.
Position Qualifications:
  • Must possess an M.S.N. degree and current Advanced Family or Adult Practice Nursing License.
  • Possess DEA license or willing to obtain one.
  • Minimum 5 years' nursing experience.
  • Demonstrated experience in counseling patients/clients.
  • Excellent interpersonal skills.
  • Ability to react quickly in emergency situations; demonstrated "street smarts".
  • Physically able and willing to work in various settings, including outdoors; this position requires riding the Health Outreach Bus for Bus shifts and vans for Street Medicine and/or the CTA.
  • Familiarity with referral sources.
  • Current driver's license and good driving record; access to reliable personal transportation.
  • Ability to work collaboratively in a team environment committed to fulfilling the mission, supporting each other, and having fun along the way.
  • Must be available evenings and weekends.

The employee must occasionally drive an automobile and the outreach bus; and lift and/or move 20 pounds.
