Hospice Nurse Case Manager
We are seeking a caring and empathetic Hospice Nurse to provide compassionate care and support to patients with terminal illnesses in La Grande, OR.
As a Hospice Case Manager nurse, you will be responsible for supervising the care of hospice patients and educating their families and caregivers on recognizing potential symptoms and providing safe and competent care. You will also provide emotional and practical support to patients, family members, and caregivers.
Hospice Nurse Responsibilities:
Supervising and overseeing the care of patients nearing the end of their lives.
Scheduling patient visits according to their needs, collaborating with physicians, managing orders and medications.
Supervising LPN's / LVN's / Hospice Aides according to the protocol.
Working with a team of medical professionals to ensure patients receive high-quality palliative care, including physical, emotional, and spiritual support.
Qualifications:
Preferably an associates degree.
Must hold a registered nurse license.
Minimum of one year of experience required.
Previous experience in hospice care is necessary.
Willingness to be on-call as required.
Must possess reliable transportation and a valid driver's license.
