Under the clinical direction of a RN, the LPN ensures that health and nursing quality standards are maintained for the individuals. This includes providing direction to the direct care staff, performing direct nursing care, and coordinating outside medical services. The LPN demonstrates knowledge of company quality mission, vision, and values. In addition, this position actively participates in quality improvement projects (i.e. communicating issues, offering suggestions) to ensure ongoing excellence of company residents, staff, and programs.

Essential Job Functions

  1. Takes direction from a RN - renders direct nursing care by examining individuals' conditions.
  2. Under the direction of a RN, observes and evaluates by seeing, hearing and using hands and fingers to observe individuals' conditions in accordance with Standard Nursing Practices and reports pertinent observations to the RN/Medical Team. Accurately documents observations, interventions and outcomes of treatment.
  3. Contacts the physician when necessary, in person, by telephone and/or in writing to discuss changes in individuals' conditions, results of consultations, lab tests, etc.
  4. Monitors accuracy and timeliness of all medication and deliveries.
  5. Transcribes and follows through on all physicians orders.
  6. Oversees the administration and accurate count and control of all controlled drugs, and ensures proper control of syringes, according to the instructions of the Bureau of Narcotics and State Bureau of Hospital Services. Prepares, administers and records medications and treatments to individuals as needed in accordance with standard nursing practices.
  7. Participates in the planning, implementation and evaluation of the plan of care for each individual.
  8. Ensures that all necessary health documentation and other requirements are up to date (clinic visits, inoculations, quarterly and annual reviews, etc.)
  9. Responsible for accurate documentation of all relevant information pertaining to the individual (e.g. nursing notes, quarterly reports, etc.).
  10. Ensures that all equipment is in good repair and functions properly. Informs Residence Manager, of equipment needs.
  11. Provides on-going direction and feedback regarding performance of direct support staff and reports issues as needed to the Residence Manager according to company policies.
  12. Acts as a liaison between other community agencies (schools, programs, clinics, families, etc.).
  13. Under the direction of an RN, observe medical needs of program participants and compiles all relevant data into a medical file/chart.
  14. Under the supervision of an RN, conducts nursing in-services annually or as often as necessary based on observed training needs.
  15. Identifies training and development needs of staff and communicates

information to Residence Manager and appropriate Workforce Development personnel verbally and/or in writing. Provides observation of staff following in-service training or re-certification, and reports findings to the RN.

  1. Meets with the nursing team, or as needed, to resolve issues and communicate relevant information.
  2. In conjunction with the nursing team, keeps current with nursing policies, practices and regulations be attending seminars, conferences and in-service training.

Accountabilities

  • Assist with staff training and development.
  • Ensure that audit documentation standards regarding clinical practice are met.
  • All other duties as assigned. Hours This is a full time, non-exempt position. Employees will be paid overtime for any hours worked over 40.

Qualifications & Minimum Requirements

  • Graduate of an accredited nursing school of nursing
  • Current New York State LPN license.
  • One to Two years nursing experience
  • One to Two years of experience working with people with developmental disabilities preferred
  • Ability to work independently and interact with all levels of staff.
