Location: Osage Rehabilitation and Health Care Center, Osage, IA
Shifts: Full-Time and Part-Time, 6 AM - 2 PM, with every other weekend required
Position Summary: As a Registered Nurse (RN) at Osage Rehabilitation and Health Care Center, you will be responsible for providing high-quality nursing care to our residents. This role involves assessing, planning, implementing, and evaluating patient care in a long-term care setting. You will work closely with other healthcare professionals to ensure the best possible outcomes for our residents.
Duties and Responsibilities:
- Patient Care:
- Perform comprehensive assessments of patients, including physical, emotional, and social needs.
- Develop and implement individualized care plans based on patient assessments.
- Administer medications and treatments as prescribed by physicians.
- Monitor and record patient progress, vital signs, and symptoms.
- Provide wound care, catheter care, and other necessary medical procedures.
- Coordination and Communication:
- Collaborate with physicians, therapists, and other healthcare team members to coordinate care.
- Communicate effectively with patients and their families regarding care plans and progress.
- Document patient interactions and care in accordance with facility policies and state regulations.
- Patient Advocacy:
- Advocate for patient rights and ensure their needs are met in a compassionate and respectful manner.
- Address and resolve patient concerns or complaints promptly.
- Compliance and Safety:
- Adhere to all regulatory requirements and facility policies.
- Ensure the safety and cleanliness of the patient environment.
- Participate in infection control practices and emergency procedures.
- Education and Training:
- Educate patients and families about health conditions and care plans.
- Stay updated with current nursing practices and regulations through ongoing education and training.
- Administrative Duties:
- Assist with admissions, discharges, and transfers of patients.
- Participate in facility meetings and contribute to policy development and review.
Qualifications:
Education and Licensure:
- Current and valid Registered Nurse (RN) license in the state of Iowa.
- Associate's Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited institution.
Experience:
- Prior experience in a long-term care or rehabilitation setting is preferred but not required.
Skills and Competencies:
- Strong clinical assessment and decision-making skills.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively as part of a healthcare team.
- Proficient in electronic medical record (EMR) systems and other healthcare technologies.
- Compassionate and patient-centered approach to care.
Additional Requirements:
- Ability to work weekends and holidays as required.
- Must pass a background check and drug screening.
- CPR and First Aid certification preferred.
Working Conditions:
- Full-Time: 40 hours per week, 6 AM - 2 PM, including every other weekend.
- Part-Time: Variable hours, 6 AM - 2 PM, including every other weekend.
- Physical requirements include the ability to lift, transfer, and assist patients as needed, and to stand or walk for extended periods.
